Patients have another way to borrow: Prosper https://www.prosper.com/landing
Prosper is America’s fastest growing online peer-to-peer lending marketplace. Encouraging people to invest in each other, Prosper facilitates the loan service on behalf of borrowers and investors.
Borrowers can apply online and list loan requests ranging from $2,000 to $35,000. In return, investors can create an account and elect to loan out as little as $25 through listings that detail a borrower’s credit scores, ratings and community affiliations along with other details. All loans are unsecured with fixed rates, making monthly payments easy for borrowers to manage.
We are also pleased to offer our patients CareCredit, North America’s leader in patient payment plans. CareCredit lets you begin your treatment immediately, and then pay for it over time with low monthly payments that fit easily into your monthly budget.

Who has access to your medical records?
The HIPAA Privacy Rule establishes national standards to protect individuals’ medical records and other personal health information and applies to health plans, health care clearinghouses, and those health care providers that conduct certain health care transactions electronically. The Rule requires appropriate safeguards to protect the privacy of personal health information and sets limits and conditions on the uses and disclosures that may be made of such information without patient authorization. The Rule also gives patients rights over their health information, including rights to examine and obtain a copy of their health records, and to request corrections. A clinical summary of your visit can be provided for you within 3 business days upon your request (hard-copy or electronic).
How old do you have to be to access medical records?
You are legally able to request access to records at age 18…. although a minor may be considered emancipated if he or she is married, has obtained court emancipation, or is a parent.
How do we keep up with where records are sent?
A data entry is created in the patient’s chart as a record of what information was sent, where it was sent, and by whom. The signed request for the records is scanned in the patient’s chart.
For all prescription refills, please call our office during office hours. A nurse will help you with your prescription refill. If you reach the nurse's voicemail, please listen carefully and leave all information requested. Please speak slowly and clearly and spell your last name. A nurse will return your call as soon as possible.

Surgery Scheduling & Pre-Certification
When surgery is scheduled, we will verify your insurance coverage and complete any necessary pre-certification. Inaccurate insurance information could result in an invalid pre-certification. Pre-certification does not guarantee payment from your insurance company. As the patient and the member of the insurance company, it is your responsibility to know if your insurance will pay for the surgery.
We ask that you be prepared at your pre-op visit to pay any insurance deductible and co-pay. If you can't pay in full, you must contact the Surgery Scheduling Department to arrange payments at least 3 days prior to your pre-op appointment.
