What to Bring with You
Be sure to bring your insurance cards and a picture identification with you, so that we can scan these documents into your electronic medical record. In order to protect your identity and to protect you from insurance fraud, we will take your photo with a camera connected directly to your electronic medical record.
You can even save some time at your first visit by printing out and completing these new patient forms in advance of your appointment.
In order to view or print these forms, you will need Adobe Acrobat Reader installed. Click here to download it.

What to Expect at your First Visit
Being well-prepared for your appointment will ensure that the doctor has all of the needed information to provide the best possible care for you. It also will help relieve any unnecessary anxiety you may be feeling. Educate yourself on your symptoms by reviewing the content on our website. Also, take some time to review our staff page and familarize yourself with the doctors.
During your first visit, we make sure to obtain important background information, like your medical history and give you time to get to know your doctor. Most initial evaluations take approximately one hour. More complex or emergency (acute) type evaluations can take up to two hours. Please allow for sufficient time for us to complete the examination and provide the best care for you.
We look forward to your first visit!
