 Your First Visit: What to Expect

When you arrive at our office, the receptionist will ask you to complete medical information forms in order for us to begin a permanent medical record for you. For your convenience, these forms are provided on our web site. This allows you the convenience of completing the necessary information before you arrive at our office.

Be sure to bring your insurance cards and a picture identification with you so that we can make copies for your file.

We will review your medical history so the doctor is aware of any health conditions, previous surgery, family history, and medications your are taking. It is helpful to bring along any medical records pertaining to your present illness and any additional information you think may be important. BRING ALL MEDICATIONS YOU ARE TAKING.

Your initial evaluation may take between one and two hours, especially if it involves more than a simple exam or an emergency (acute) type problem. Please allow for sufficient time for us to complete the examination.
